About Us

EIR AXE develops healthcare programs that connect evidence-informed nutrition and behavioral approaches with practical delivery and measurement. We focus on the work between a promising intervention and an operating program: referral, enrollment, benefit design, food sourcing, fulfillment, engagement and follow-up.

Our approach draws on earlier field work led through Synergy HealthTech. With payer, clinic, housing, research and food-distribution partners, the historical programs served Medicare and Medicaid populations and delivered more than 100,000 healthy meals across multiple cohorts. That work also informed Kabixa, a functional benefit facilitation prototype, and Field & Yield food-supply workflows.

Today we apply those lessons to rural health, preventive care and senior-care opportunities. Current PACE and related work is in development and requires qualified clinical, operating and institutional partners.
